Transitions

Transitions is a new dynamic program at Health Initiative for Men that engages a diverse group of men involved in the sex industry in Metro Vancouver. Through a multi-phase program of support, Transitions aims to increase men’s personal and professional capacity and provide them with the tools necessary to reduce their reliance on sex work so that they can transition, retire, or exit from the sex industry.

Supported by HUSTLE and the Transitions team, program participants will have the opportunity to identify their own personal strengths and challenges, set goals, and engage in one or more of the following phases:

PRE-ENGAGEMENT

In the Pre-Engagement Phase, potential participants may self-refer, be referred by HIM/HUSTLE and/or externally from community into Transitions. They will meet with the Service Coordinator and the Transitions Team to receive the following support:

  • recruitment into the program;
  • schedule intake interveiws;
  • one-on-one case management;
  • referrals to internal/external groups, programs, services;
  • wrap-around support to build constructive relationships and networks among participants

PHASE ONE:

In Phase One, potential participants will work with the Service Coordinator and the Transitions team to receive the following support and services:

  • attend intake interviews;
  • participate in needs assessement ;
  • one-on-one case management;
  • referrals to internal/external groups, programs, services;
  • wrap around support.

PHASE TWO:

In Phase Two, Transitions participants will work with the Service Coordinator and the Transitions Team to receive the following support and services:

  • ongoing case management and one-on-one support
  • life skills and capacity building workshops;
  • referrals to internal/external groups, programs, services
  • review care plans;
  • wrap around supports. 

PHASE THREE:

In Phase Three, Transitions participants will work with the Service Coordinator and the Transitions Team to receive the following support and services:

  • ongoing case management and one-on-one support
  • pre-employment and supported employment skills workshops;
  • referrals to external groups, programs, employment services, education and/or training institutions;
  • address remaining goals;
  • graduation ceremony.

POST-ENGAGEMENT PHASE:

In the Post-Engagement Phase of the Transitions Program, participants will work with the Service Coordinator and the Transitions Team to receive the following support and services:

  • individualized after-care plans to ensure continuity of care;
  • referrals/accompaniments to relevant community resources as identified in the individualized after-care plan;
  • monthly after-care meetings with peers to share support, experiences, successes and challenges;
  • ongoing check-ins with program graduates to assist with continuity of care and support.
